The PI3K-AKT-mTOR axis is a master regulator of cell growth, metabolism, and survival, and it ranks among the most commonly activated networks in human cancer. Receptor tyrosine kinase engagement recruits and activates class I phosphoinositide 3-kinase, whose regulatory subunit is detected as PI3K p85; the catalytic activity generates the lipid second messenger PIP3 at the plasma membrane. This lipid is directly antagonized by the tumor suppressor lipid phosphatase PTEN, which dephosphorylates PIP3 and restrains pathway output. PIP3 recruits AKT (pan), which is activated by sequential phosphorylation, including at serine 473 detected as Phospho-AKT S473, by PDK1 and the mTORC2 complex. Active AKT phosphorylates numerous substrates that promote survival, glucose uptake, and biosynthesis, and it relieves inhibition of the kinase mTOR. Within mTOR complex 1, active mTOR, detectable as Phospho-mTOR, phosphorylates the translational regulators p70 S6K and 4E-BP1; phosphorylation of 4E-BP1 releases eIF4E to drive cap-dependent translation, while p70 S6K promotes ribosome biogenesis and protein synthesis. In cancer, this pathway is hyperactivated through PI3K mutation or amplification, loss of PTEN function, and AKT activation, fueling proliferation, metabolic reprogramming, and resistance to apoptosis and targeted therapies. These features have driven the clinical development of PI3K, AKT, and mTOR inhibitors, whose effects are best monitored through pathway phosphorylation readouts. Assessing PI3K p85, PTEN, total and phospho-AKT, total and phospho-mTOR, and the downstream effectors p70 S6K and 4E-BP1 provides an integrated measure of pathway activation and drug response.
