The extrinsic apoptotic pathway is initiated at the cell surface when death ligands engage their cognate receptors, transmitting a death signal directly from the extracellular environment into the cytosolic proteolytic machinery. Binding of ligands such as TRAIL to the death receptor DR5, or engagement of Fas / CD95 by FasL, induces receptor trimerization and a conformational change in the cytoplasmic death domains. This nucleates assembly of the death-inducing signaling complex, in which the adaptor FADD bridges the clustered receptors to initiator caspases through homotypic death-effector-domain interactions. Recruited Caspase-8, and in humans Caspase-10, undergo proximity-induced dimerization and autoprocessing, generating active proteases that either directly cleave executioner caspases in type I cells or, in type II cells, cleave BID to a truncated form that engages the mitochondrial pathway and amplifies the death signal. The intensity of this cascade is tuned by cFLIP, a catalytically inactive caspase-8 homolog that competes for recruitment to FADD and blocks initiator caspase activation. In cancer, dysregulation of this pathway promotes immune evasion and treatment resistance: tumors frequently downregulate DR5 or Fas / CD95, overexpress cFLIP, or acquire defects in FADD and Caspase-8, thereby escaping cytotoxic lymphocyte surveillance that operates partly through death-ligand engagement. Conversely, the selectivity of TRAIL for transformed cells has motivated agonist antibodies and recombinant ligands aimed at DR5 to reactivate extrinsic killing.
